I swear if this grows any slower I am going to go mad! Now for my question. Should I pinch it back and if so to where?

Hi DW, If you intend to grow it on as a 'standard' carefully remove that left side shoot, reason for this is at the moment some of the 'growth-energy' is going to same instead of all the energy going to the main leader.

Also remove that small/short cane and replace with a longer/thicker one. Always have the cane taller than the plant, otherwise you run the risk of the wind 'snapping-off' the stem, which would be a shame. Loosely tie in the main stem. This will give more support and keep main stem straight.

As it grows remove any lower side shoots, but leave on any leaves that are growing directly out of main stem, these give strength to main stem and will fall off naturally.

Always leave the top 3 side shoots on the main stem, removing the lower pair as more form on the top, this way if you accidently damage the main growing tip you still have the top side shoots which will later form the 'head' of the standard.

Now let's see it again when it's 3ft high

It's done, in three years when it actually makes it that big I will be sure to share it LOL. Is there anything I can do to promote growth? And still keep a nice strong root base? I did give it some fish emulsion last weekend
